Care Management Program for the Uninsured and Underinsured in Orange County
The Foundation provided support to Florida Hospital Healthcare System Care Management division to provide support and education services to diabetics who are referred from the Emergency Room and the PCAN clinics located at Alafaya Trail and Hoffner. The clinics are operated by Central Florida Family Health Centers. It was focused on uninsured Hispanics in Orange County.
Improving Community Health
The Foundation-supported project at the Orange County Health Department was designed to support quality improvement performance management approaches at the Orange County Health Department
Pharmacy Project Patient Assistance Programs
WPHF provided support for a second year for three Patient Assistance Program Coordinators who work in collaboration with the providers and other health care personnel to obtain medications available under the Patient Assistance Program (PAP). The coordinators help patients and families to understand, accept and follow eligibility requirements and limitations of the PAP programs. Community partners include the Health Care Center for the Homeless, the Center for Drug Free Living and the Central Florida Family Health Center.
Shepherd's Hope Pharmacy Case Manager
WPHF provided support to enable Shepherd’s Hope to hire a program case manager to help clients access the PAP programs and navigate the healthcare system.
Health Navigators
The Foundation continued to fund multi-lingual health navigators to work with uninsured residents to locate the most appropriate programs and options to provide health care.
Central Florida Regional Health Information Organization (CFRHIO)
WPHF provided support for the planning phase for a working Regional Health Information Organization (RHIO) model that will demonstrate information technology connectivity among health care providers and long-term sustainability through user fees.
Enhancing Health Care through Electronic Health Records Health Care Center for the Homeless
WPHF has supported the Health Care Center for the Homeless efforts to build organizational capacity through the implementation of an electronic health record system. The goal is to strengthen organizational efficiency and improve the quality of service delivery.
CommonSense Childbirth
The Foundation provided funds to CommonSense Childbirth to offset the cost of providing prenatal services to uninsured women at its midwifery clinic and birthing center located in Winter Garden. It serves a predominantly African-American population at risk for delaying prenatal care due to lack of resources or support.
Medicaid Research Conference
Funds were provided to help sponsor the 5th Annual Florida Medicaid Research Conference on Patient Empowerment and Health Literacy hosted by the Agency for Health Care Administration & Florida Center for Medicaid and the Uninsured at the University of Florida.
Annual Askew Institute Meeting
University of Florida
WPHF also helped sponsor the Annual Askew Institute Meeting-- "Recent Policy Changes in Medicaid and SCHIP - What Works and What Doesn't?"
KidCare Outreach
Recognizing the importance of educating residents about the KidCare insurance coverage program, the Foundation provided funds to promote the program in the community.
Volunteer Appreciation
WPHF funds supported Volunteer Health Care Provider Appreciation recognition.
Florida Health Policy Center
Foundation funds helped support the costs of a feasibility study to determine the need for a health policy center in Florida.
